We’ve made getting insurance as easy as 1-2-3. When you need healthcare coverage but aren’t eligible for traditional Medicaid, Medicare or CHIP, CountyCare can help.
CountyCare is a new Medicaid program for uninsured adults in Cook County that offers expanded coverage under the new health care reform law, the Affordable Care Act. It is operated by the State of Illinois and the Cook County Health and Hospitals Systems (CCHHS).
1. Who can join
To join CountyCare, you must:
+ Live in Cook County
+ Be 19-64 years of age
+ Be a legal immigrant for at least 5 years or be a US citizen
+ Meet income limits ($15,282 for an individual or $20,628 for a couple)
+ Have a Social Security Number or have applied for one
If you qualify for Medicaid, Medicare or CHIP, you cannot join CountyCare.
2. What is covered
Here is a list of the services covered under CountyCare’s extensive plan:
+ ER visits
+ Hospital inpatient services
+ Hospital outpatient services
+ Physician services
+ Sub-acute alcohol and substance use disorder services
+ Mental health (including rehabilitation and clinic option)
+ Nursing facility care (30 days - covers post-hospitalization nursing home stays)
+ Medical supplies and equipment, prostheses and orthoses, and respiratory equipment and supplies
+ Home health agency visits
+ Advanced practice nurse services
+ Hospice and palliative care
+ Laboratory and x-ray
+ Rehabilitation services, including physical, occupational, hearing and speech therapy
+ Prescription drugs
+ Bus vouchers for covered services
+ Family planning and supplies
+ Dental (for 19-20 year olds)
+ Podiatric services for diabetics
+ Targeted Case Management (behavioral health)
+ Early and Periodic Screening, Diagnostic, and Treatment (EPSDT) (for 19-21 year olds)
+ Federally qualified health center (FQHC) visits
3. How to apply

Call us at 312.633.5841 between 8 a.m.-6 p.m., Monday-Friday to schedule an enrollment appointment at your nearest PrimeCare health center.
You’ll be asked to bring a list of documents you will need to bring to your appointment.
Individuals may apply at PrimeCare, a CountyCare participating FQHC, located at 1431 North Western Avenue, Suite 406, in the Presence Saint Elizabeth Hospital Medical Office Building, between 8 a.m.-6 p.m., Monday-Friday.
If approved, you will receive a welcome packet which includes your CountyCare Medical Card and information packet in the mail. Keep this card with you at all times. If you are denied approval, you will receive a letter with appeal instructions.
Finding a Doctor In-Network
When you join CountyCare, you will have an opportunity to choose your primary care site from a list of CountyCare providers (Cook County clinics and federally qualified health centers). If you already have a doctor, confirm that your doctor is in the CountyCare network before making an appointment. Not all Medicaid doctors see CountyCare patients.
